2100-98 Kansas Probate Forms 3d 2177 (CAPTION) ORDER REVOKING OUTPATIENT TREATMENT AND COMMENCING INPATIENT TREATMENT NOW, on this day, the Court holds a hearing to determine whether the previously issued outpatient treatment order should be revoked. The patient appears in person and by patient’s attorney, W.B. Pleader. The petitioner appears in person and by John Smith. After hearing all the evidence, statements and arguments of counsel, the Court finds by clear and convincing evidence that Betsy Doe violated the conditions of the outpatient treatment order entered on July 22, 2010, by failing to keep regular appointments with her assigned caseworker and failing to take prescribed medications. The Court further finds that it is in the best interests of said person to be treated at Oakwood State Hospital. THE COURT ORDERS: 1. The outpatient treatment order entered on July 22, 2010, is revoked and the patient shall receive inpatient treatment until October 22, 2010, which date is the end of the previously set period of treatment, subject to further order of the Court. 2. The head of the treatment facility then furnishing treatment to the patient shall submit to the Court a written report summarizing the treatment provided and the findings and recommendations of the treatment facility concerning the need for further treatment not later than October 8, 2010. 3.
